Description
As part of the Child & Family Community Integrated Care (CFCIC) the Transitions Coordinator will support the development and implementation of a transition framework for adolescents and young adults (AYA) with complex medical needs as they move from pediatric to adult healthcare teams. Working in collaboration with the Clinical Manager for Complex Care and broader transition working groups, the Coordinator will focus on the system level processes required to ensure safe, equitable and developmentally appropriate transitions from pediatric to adult healthcare teams. The coordinator will work collaboratively with internal pediatric and adult service providers to create a scalable model for transitions, informed by best practices, quality metrics, and patient/ family engagement.
1. Regulated Health Professional in good standing.
2. Undergraduate degree in health sciences or equivalent
3. Minimum 2 to 5 years clinical and / or systems level experience, preferably in pediatric or AYA-focused care.
4. Proven leadership in clinical coordination or care transitions.
5. Experience working with medically complex youth and inter-professional teams.
6. Strong knowledge of AYA transition frameworks and community care systems.
7. Proficiency using software programs (Microsoft Word, Excel, PowerPoint)and familiarity with EMR systems (EPIC)
8. Ability to use office equipment (fax machine, teleconference equipment, projector, etc.)
9. Knowledge of study design and data collection, analysis and dissemination.
10. Knowledge of the principles of Quality Improvement and ability to apply these.
11. Proven ability to deliver under tight deadlines with effective time management skills
12. Experience participating in teams to successfully achieve outcomes of a complex nature
13. Leadership, facilitation, mentoring, dispute resolution & relationship building skills
14. Skills of a “Self-starter” who can work with limited guidance
15. Well-developed analytical and problem solving skills
16. Excellent Verbal and written communication skills
17. Excellent presentation skills and use of power point
18. Commitment to equity, inclusion, and family-centered care
