Description
As a member of the inter-professional pediatric chronic pain team, the Social Worker will deliver care to children, youth and families in an outpatient tertiary care setting. The Social Worker will promote excellence in pediatric patient and family-centred care through expert clinical practice, education, consultation and research. Responsibilities will include: providing social work assessment and treatment as part of an inter-professional team; providing evidence based treatment or best-practice therapies; managing and maintaining an active caseload as per program requirements and expectations; providing case management for patients and families; working collaboratively with other team members to develop and implement comprehensive individualized care plans for children and adolescents; working with patients and families in developing treatment pathways that best support optimal functioning; consultation/liaison to caregivers and community agencies; and participating in program development initiatives and quality assurance projects.
1. Master's Degree in Clinical Social Work from an accredited School of Social Work
2. Current registration with the Ontario College of Social Workers and Social Service Workers
3. Minimum of 3 years clinical experience in a tertiary pediatric mental health setting
4. Demonstrated expertise in assessment, psychoeducation, crisis intervention and advocacy skills in working with children, teens and their families experiencing health and mental health issues affecting child and family functioning
5. Demonstrated expertise in a range of evidence based, therapeutic modalities (i.e. cognitive behaviour therapy (CBT), motivational interviewing (MI), Emotion Focused Family Therapy (EFFT), Acceptance and Commitment Therapy (ACT), Dialectical Behavior Therapy DBT, mindfulness, family therapy for the assessment, treatment, and support of children, youth and their families in a pediatric setting
6. Demonstrated knowledge with the Mental Health Act, Consent to Treatment Act, Child and Family Services Act
7. Demonstrated excellent interpersonal communication skills, critical thinking, mentoring and coaching within an interdisciplinary team9. Demonstrated ability to manage an active caseload and knowledge of community resources and supports
8. Demonstrated knowledge and ability to provide suicidal risk assessment, crisis management and verbal diffusion techniques
11. Demonstrated ability to work independently as well as collaboratively within an interdisciplinary care model
12. Demonstrated facilitation, coordination, assessment, planning, implementation and evaluation skills
13. Evidence of commitment to ongoing continuing education of self and others
14. Willingness to participate in student education as appropriate, including providing Field Instruction to Social Work students
15. Willingness to engage in clinical research within the interdisciplinary team
16. Relevant pediatric chronic pain experience preferred
