The Hamilton Niagara Haldimand Brant (HNHB) RLT Coordinator will be a team member of the HHS Radioligand Therapy Team.

The position will work collaboratively with other HNHB Radiopharmacutical and radioligand therapy community members (RLT Coordinators, Nuclear Medicine physicians, medical oncologists, Radiation Safety Officers) to receive and review all referrals for Radioligand Therapies at HHS which includes PLUVICTO.

Key accountabilities include:

- Interpretation of clinical information provided post MCC to utilize in the Nuclear Medicine referral pathway. Identifying concerns for patient safety and readiness to treat

-          Communication with relevant stakeholders (patients, family members, medical oncology, nuclear medicine, vendor and PSP supports)

-          Received and action inquiries regarding RLT programming, monitoring ongoing therapy eligibility

-          Coordinate enrollment with Patient support program

-          Work collaboratively with multi disciplinary teams to ensure and coordinate seamless transitions in patient treatment

-          Liaise with clinical teams regarding recommendations, patient requests and ready to treat orders

-          Manage patient appointment while ensuring individual rights and patient advocacy are maintained using the “time to treat” and “care close to home” principles

-          Provide guidance and education to referring sources regarding referral processes and services

-          Participate in learner education where appropriate

-          Maintain an accurate and current database of referrals, outstanding orders in “ROME” (Radiopharmaceuticals Order Management Environment)

-          Follow established protocols and priorities for urgent referrals.

-          Escalation of required POs to mitigate dose waste and cancellations.

-          Liaise with Decision support to aid in quarterly data submissions

-          Report adverse reactions to RLTs as per standards

-          Participate in quality assurance activities ensuring accuracy of data and information


Qualifications

- Undergraduate Degree - Health related discipline

· Strong understanding of Radioligand Therapies, Nuclear Medicine and care planning strategies

· Decision-making with a high degree of independence and strength in problem solving and accomplishing priorities

· Strong organizational skills with a demonstrated ability to manage multiple tasks, activities and priorities

· Demonstrated leadership and team work skills

· Excellent communication (verbal and written) and team building skills

· Exceptional change management, facilitation and coaching skills

· Strong knowledge base of Quality, Risk Management.

· Excellent interpersonal, diplomacy, critical thinking and attention to detail

· Demonstrated ability to facilitate effective working relationships with internal and external customers and stakeholders,

   including the ability to negotiate and resolve conflict

- Demonstrate computer skills: advanced experience in MS Word, PowerPoint, Excel, Access and other appropriate programs
