The Nursing Practice, Education and Community Health Portfolio includes population health, seniors' strategy, ambulatory, complex and palliative care, and contributes to system wide transformative change. As part of this portfolio, the Geriatrics and Supporting Services Program has a specialized focus on geriatric care and enterprise-wide clinical services including skin, wound and ostomy care, and vascular access therapy, providing expert internal and external support to acute and community providers, and is a lead in supporting the senior friendly care strategy across HHS.

The Geriatric Rehab Unit (GRU) is comprised of 18 beds which cares primarily for frail seniors with a variety of complex medical comorbidities. These older adult patients require a period of rehabilitation prior to discharge from hospital.

As a vital member of the healthcare team, the Physiotherapist coordinates, assesses and evaluates patient care and provides physiotherapy care to meet the needs of patients. The PT will work closely with a Physiotherapy Assistant. The Physiotherapist collaborates with interdisciplinary team members and community agencies in the provision of care and discharge planning for patients and their families. The Physiotherapist practices in accordance with the College of Physiotherapist of Ontario Standards of Practice and Guidelines and according to Hamilton Health Science Policies and Procedures. The Physiotherapist utilizes a patient and family centered care approach and incorporates best available evidence into practice. The Physiotherapist participates and provides clinical education to physiotherapy students and others through preceptor, tutor and/or clinical lecturer activities. In addition to providing clinical care, the Physiotherapist actively participates in continuing professional development opportunities, professional practice and program committees&/initiatives that support patient care, clinical practice, education and research as approved by Clinical Manager in conjunction with the Chief of Physiotherapy Practice. Some coverage to other clinical areas may be provided as needed.
