Description
The ambulatory oncology pharmacist oversees the medication management system and provides direct patient care to identify, solve, and prevent drug therapy problems using evidence-based practice. The pharmacist provides patient education, teaches learners, provides drug information and education to nursing and medical staff, and ensures patient medication safety by supporting drug distribution services. As part of the interdisciplinary team, the oncology pharmacist builds relationships and communicates with peers across the organization including physicians, nurses and staff. Finally, the oncology pharmacist reinforces HHS’ four corporate pillars: improving the care of our patients, engaging fellow staff, stewarding precious healthcare resources and contributing to research, innovation and learning.
Responsibilities include:
• Providing patient-, and treatment-centered clinical services including chemotherapy medication order review and assessment, medication reconciliation, inter-professional participation, adverse drug reaction reporting, medication counseling, medication incident reporting and follow-up
• Helping manage our cutting-edge CPOE-based medication management system, along with oversight of our formulary, inventory control and drug procurement processes
• Participating in drug information and other educational and teaching activities within the Centre in collaboration with our nursing colleagues
• Participating in continuous quality improvement, research and patient safety initiatives
• Collaborating with our pharmacy technicians to ensure timely and accurate chemotherapy compounding
• Following all Hospital and governmental (Federal and Provincial) rules, regulations and policies set to control the dispensing to the general public and the distribution of medications, and to provide high quality services at the Centre
• Graduate from an accredited university Pharmacy Program
• Licensed under Part A of the Ontario College of Pharmacists
• Experience in ambulatory/inpatient oncology setting is preferred
• Pharmacy Residency is an asset
• Knowledge of pharmaceutical care process, disease management and clinical pharmacology
• Familiarity with evidence-based practice
• Demonstrated skills in computerized pharmacy systems
