Nurse Practitioners (NP) provide leadership and promote excellence in comprehensive care through the five main role components of expert practice, education, consultation, research, and leader/change agent. This NP role will focus care delivery with the Sickle Cell Disease and Thalassemia patient population. Nurse Practitioners play an integral role in delivering comprehensive, collaborative, and integrated support for patients with hemoglobinopathies, across chronic and acute episodes of care. Key components include working as part of a team, clinical care and expertise, patient and family support, leadership and quality improvement, and professional development. 

Role Description 
• Develop broad clinical skills and experience in hemoglobinopathies to lend support to this growing and expanding clinical service 
• Develop and utilize specialized clinical expertise and in-depth knowledge as you will provide comprehensive care to a defined population within the Hemoglobinopathy Program
• Perform patient assessments and coordinate an interdisciplinary plan of care for patients with Sickle Cell Disease and Thalassemia in the outpatient clinic setting 
• Collaborate with the Medical Day Care nursing team and Transfusion Medicine team to provide comprehensive care to transfusion-dependent patients 
• Develop care plans with patients, families, and the interprofessional team, including pain management plans.
• Monitor patients’ responses to their care plans, modifying as needed
• Recognize the influence of social determinants of health on care and collaborate with the team to facilitate access to appropriate support, services and resources in the community 
• Partner with patients and families to navigate their chronic illness and advocate for their needs 
• Provide relevant health education to patients and families that is based on their needs, learning styles, and stages of development
• Lead the assessment, planning, education, and support of adolescent patients transitioning from pediatric to adult care, developing individualized transition plans in collaboration with families and healthcare teams
• Support the implementation of the Ontario Health Quality Standards for Sickle Cell Disease
• Apply evidence-based research to clinical practice and quality improvement initiatives
• Assume a leadership role in influencing clinical nursing practice and the provision of care 
• Integrate research into practice, contribute to advancing the quality of clinical practice
• Demonstrate a commitment to lifelong learning and apply principles of change theory to support personal and team development


Qualifications

• Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse Master’s Degree in Nursing or equivalent with a clinical focus or completion of a Post-Master of Nursing (NP Field) Diploma 
• Extended Class, in good standing 
• 3 or more years of clinical nursing experience in direct patient care 
• Excellent communication and relational skills 
• Innovative critical thinking with excellent problem-solving skills 
• Excellent time management skills 
• Excellent written and verbal communication skills
• Ability to work both independently and part of an interdisciplinary team
• Demonstrated commitment to equity, diversity, and inclusion (EDI) and experience advancing EDI objectives 

Assets  
• Experience working with the hemoglobinopathies patient population
• Experience in Hematology and/or Pediatrics
• Ability to act as a change agent, and foster teamwork and consensus-building 
• Previous experience in a leadership role
• Commitment to life-long learning to grow NP practice 
• Knowledge of theoretical frameworks and research application 
• Ability to provide trauma-informed counsel and education to patients and their families in Hemoglobinopathy health related topics
• Knowledge and experience in anti-racist frameworks of care
• Experience with chronic health maintenance including complications across the lifespan and transitioning pediatric patients to adult care is an asset
