Description
The Nurse Navigator (NN) will work collaboratively as a multidisciplinary member of the team to triage referrals and coordinate the services and subspecialties required for women with complex health concerns related to endometriosis. This role will assist women to access timely quality care, organized for maximum efficiency and effectiveness. The NN will remain a consistent point of contact for the woman along the continuum of her care, from initial assessment to post treatment or procedure follow up. The NN will participate in education and research activities, as well as quality initiatives, that contribute to the development and maintenance of health care systems which promote improved health outcomes.
1. Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse
2. Current BCLS certification
3. Minimum 5 years recent (within the past 2 years) clinical experience in a Women’s Health setting, including expertise with cultural diversity
4. Current knowledge of the physiological, psychosocial and emotional needs of women diagnosed with endometriosis
5. Successful completion of a recognized Well Woman course within the past 5 years
6. Demonstrated expertise in patient & family centered care and patient advocacy
7. Demonstrated understanding of diagnostics used in assessment of endometriosis
8. Demonstrated expertise in patient assessment, teaching and support
9. Demonstrated leadership experience within a multidisciplinary team
10. Demonstrated competency in the use of Meditech, EMR, Sovera, Microsoft Office and web based computer applications
11. Demonstrated knowledge and experience in adult education and presentations to groups
12. Demonstrated excellence in critical thinking and problem solving
13. Demonstrated excellence in interpersonal and communication skills and role modeling of HHS Values and Code of Conduct
14. Demonstrated organizational and time management skills, and ability to be self directed
