Manager - Regional Geriatric Program Central

Reporting to the Director, Geriatrics and Rehabilitation, and in collaboration with the Regional Geriatric Program Central Chair, the Manager of the Regional Geriatric Program Central provides leadership of the Regional Geriatric Program Central, and manages the delivery of high quality, evidence-based, older adult -focused, and fiscally responsible care or service that is consistent with the corporate and program strategic direction. The Manager is responsible for providing leadership, system development, change management and clinical expertise for the ongoing development and implementation of this regional program.

  1. The Manager of the Regional Geriatric Program Central in collaboration with the Director and Chair; manage and co-ordinate the development and implementation of the Regional Geriatric Program Strategic plan and implementation of the program for Hamilton, Niagara, Haldimand, Brant, Burlington and Waterloo Wellington Regions.
  2. Provide clinical leadership and program management of the Centralized Clinical Intake for Specialized Geriatric Services and Behavioural Supports Ontario.
  3. Provide program management and change management leadership in the development and implementation of programs including research to the field guiding by best practices and core competencies.
  4. Lead related committees and liaise with appropriate Regional Geriatric Program community stakeholders and academic and research institutions.
  5. Provide leadership, management and supervision of the Education Program inclusive of the Geriatric Certificate Program.
  6. Partner and be a leader in proposal submissions and negotiation for funding opportunities, legal/contractual agreements and purchase of service agreements for health professionals, researchers and students.
  7. Assist and work in collaboration with the Regional Geriatric Program Central Chair, lead physicians and Ministry representatives in securing retention efforts for the recruitment of medical professionals in the field of geriatrics.
  8. Manage the financial, human and physical resources of the program in accordance with the annual business plan; provide leadership, supervision and direction to employees of the program.
  9. Prepare the current budget estimates and monitor expenditures in collaboration with the Regional Geriatric Program Central Chair and HHS Finance.
  10. Prepare and execute the annual business plan that will detail goals, expected service/performance outputs, resources required, and the performance measures used to assess the program’s performance against the goals.
  11. In partnership with the Regional Geriatric Program Central Chair and regional stakeholders, review, evaluate and monitor the delivery of specialized services to align geriatric care and best practices across the Hamilton, Niagara, Haldimand, Brant, Burlington , and Waterloo Wellington region.
  12. Support the Regional Geriatric Program Council.
  13. Liaise with all levels of government to increase the capacity of the delivery of the programs and services being offered by Regional Geriatric Program Central.
  14. Participate in the accreditation process and work to ensure that the program achieves, maintain and continually improve upon their accredited status.
  15. Mentor, coach and lead staff in the development and monitoring of quality indicators and improvement initiatives.
  16. Perform other duties as required. 

Qualifications
  1. Post-secondary education, preferably at the Masters level, in a related field, preferably gerontology and geriatric care.
  2. Additional degrees/certificates in the field of Gerontology; Adult Education; Program Management, Business Management.
  3. Success with developing interagency collaboration in service delivery.
  4. Development of Memorandums of Agreement/Understanding/formal partnership agreements.
  5. Clinical knowledge: assessment, planning and intervention related to the focus of older adults living with frailty.
  6. Experience in project management, policy development and program evaluation.
  7. Demonstrated effectiveness in building partnerships with a broad range of individuals and organizations of diverse backgrounds within the health and social service sectors.
  8. Current experience with strategic planning, financial management including budget development, change management, demonstrated leadership in system development and knowledge of legislation, and developing teams.
  9. Results oriented with demonstrated leadership in system development, planning and implementation, conflict resolution, negotiation, interpersonal and team building skills
  10. Strong analytical and critical thinking skills, with the ability to respond to emerging opportunities
    and risks.
  11. Proven written and verbal communication skills, including effective presentation skills.
  12. Proven ability to adapt readily to change with strong organizational and planning skills.
  13. Five to seven years’ experience working directly with older adults living with frailty; in a health or social service environment including community.
  14. Minimum of five (5) years’ experience in a senior clinical leadership position.
  15. Strategic leadership and administrative experience at the system level.
  16. Training and experience with financial management and budget development.
  17. Evidence of successful proposal submissions; clinical services and research grants
  18. Demonstrated leadership in system development, planning and implementation.

  10. Demonstrated ability to respond to emerging opportunities and risks.
  11. Demonstrated ability to align systems to facilitate better service for older adults served by the Regional Geriatric Program Central.
  12. Demonstrated ability to coach, advise and mentor team members and partners utilizing the principles of adult learning.
  13. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  14. Knowledge of relevant legislation and principles of management, hospital committee structure, conflict management and resolution, system needs analysis and organizational assessment.
  15. Ability to use tact and discretion in dealing with health care providers and employees.
  16. Demonstrated excellent computer skills with proficiency in Microsoft Office software, e.g. Word, Excel, Power Point and Outlook and patient information systems.
