Description
The CNS will utilize her/his advanced knowledge of the neurosurgical patient population and program development skills to assess, analyze, develop, implement, and evaluate a regional model of care. The regional model of care will support the transition of neurosurgical patients across the continuum from acute care to the community through capacity building, collaboration, and partnerships. The Neurosurgery Outreach Coordinator Clinical Nurse Specialist (CNS) will work in partnership with the physicians and other members of the inter-professional team.
In addition to the above focus, expected outcomes for this new advanced practice nursing role include; working in partnership with other care navigators/coordinators in rehabilitation services and the acute care system in order to ensure the right level of care is provided at the right place, right time and by right provider; development of a mentorship model for skilled HHS ICU nurses and community based nurses - building capacity to smooth the way for continuum of care; development of triage criteria and processes for neurosurgical patients; provision of clinical care to complex neurosurgical patients and assess readiness for rehabilitation services; and participation in and taking the lead for research and quality improvement initiatives to advance neurosurgical patient care practices and the overall patient experience.
This position will report jointly to the Program Director - Neurosciences, and the Chief of Nursing Practice.
2. Masters Degree in Nursing
3. Membership in provincial, federal or international professional associations
4. Recent (minimum 5 years) clinical practice within the last 8 years
5. Experience in neurosurgical nursing within the last 3 years
6. Demonstrated knowledge and understanding of community services and resources for the neurosurgical population
7. Ability to develop partnerships and strategic plans
8. Experience in clinical research is an asset
9. Demonstrated ability for innovation, independent action initiative, problem solving, organization and prioritization
10. Demonstrated excellent interpersonal and communication skills
11. Demonstrated ability to promote professional development and team building
12. Demonstrated commitment to patient and family-centered care
13. Demonstrated ability to contribute to program design and evaluation, including the identification of outcome indicators
14. Demonstrated ability to design and evaluate outcome measures
15. Demonstrated commitment to ongoing professional development
16. Formal leadership experience preferred
17. Eligible for faculty appointment
18. Ability to establish linkages with CNS' in other parts of the region
