This PT RN Case Manager role will provide coverage in the Palliative Care, Medically Complex, Restorative and Behavioural Programs.  
In this leadership role the Case Manager is responsible for supporting  admissions & discharges; coordinating the timely completion of the MDS Assessment Tool for a caseload of patients; ensuring continuity of care for a caseload of patients, which may include the provision of hands-on care such as ADL, transfers, bathing, wound management, management of IV & vascular access; managing patient & family concerns in a manner which supports patient focused care and problem-solving; promoting collaborative practice which includes coordinating care among team members; integrating the components of evidence based practice to improve the quality of patient care; contributing to program development including participation in Program CQI projects,  managing  resources in a fiscally responsible manner; and mentoring of nursing team members. The position reports directly to the Clinical Manager, Behavioural Health, for all functions pertaining to service operations and team functions on the assigned units.

 


Qualifications
  1. Graduate of an approved School of Nursing and currently registered with the CNO; BScN. preferred
  2.  Demonstrated experience in effective case management 
  3. Demonstrated skills in program development
  4. Demonstrated motivation to evaluate quality and cost
  5. Demonstrated strong assessment skills for patients and families with complex needs 
  6. Demonstrated ability to coordinate the care of patients with complex needs with other team members (including family) and community agencies
  7. Demonstrated ability to work effectively with a variety of health care professionals onsite & in the community  
  8. Demonstrated commitment to providing care that is based on documented best practice
  9. Demonstrated leadership, decision making abilities and able to take acceptable risks
  10. Demonstrated ability to adjust quickly to changing conditions - both patient and environmental conditions 
  11. Demonstrated responsiveness to challenges and ability to work well under pressure 
  12. Demonstrated commitment to teaching and life-long learning
  13. Demonstrated commitment to teaching and preceptorship of nursing students helping them to develop the nursing leadership skills
  14. Ability to attend work regularly as demonstrated by an acceptable attendance record
  15. Demonstrated exceptional communication and interpersonal skills, initiative and self-confidence
