Detailed Overview

Provides nursing consultation to adults with developmental disabilities and complex needs who are supported in the community by Community Living British Columbia (CLBC); collaborates in a variety of partnerships with external service providers, family members and other healthcare professionals in order to facilitate care planning for individuals with multiple acute, chronic medical, surgical, rehabilitative and/or psychiatric needs using a client-centered approach.
Responsibilities


  1. Provides consultation to families, advocates, service coordinators, service providers and other healthcare professionals such as physicians and nurses to support individuals with developmental disabilities.
  2. Conducts comprehensive nursing assessments through fact finding, data collection, interviewing and researching sources to determine client's health needs, service provider and caregiver learning needs and client service priorities.
  3. Initiates and develops an individual health care plan for the client and/or evaluates and modifies existing health care plans, where appropriate with the caregiver, family and/or other members of the healthcare team.
  4. Designs and develops individualized health care protocols for specific health needs and issues such as procedures, approaches, emergency action plans, recording and reporting tools, community health resource information, reference material and information about medical equipment and supplies in order to provide direction to caregivers/service providers in the delivery of client care.
  5. Develops and implements effective recording, reporting and other administrative systems and protocols to promote HSCL program efficiency; facilitates client's access to medical care and healthcare services in their local community; provides information regarding policies, guidelines and regulations applicable to the client's residential and health care needs.
  6. Identifies individual learning needs of service providers, caregivers, families, advocates and other healthcare professionals as they relate to health issues and developmental disabilities; participates in their ongoing education and evaluation of training needs to meet care planning needs of the client.
  7. Initiates and/or participates in multi-disciplinary care conferences; liaises with clients, families, advocates, service providers and other healthcare professionals to ensure effective care planning and implementation of services.
  8. Participates in quality improvement activities and research by identifying client care issues and collecting data; develops recommendations regarding areas for improvement and forwards information to the Manager for follow up; participates in the development of program resources, services, standards, policies and guidelines on an ongoing basis.
  9. Participates in staff education programs, assists with orientation of new staff, students and other health care professionals.
  10. Performs other related duties, as assigned.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years' recent related clinical nursing experience in an acute care, chronic care and/or community health setting working with adults with developmental disabilities including one (1) year experience in a supervisory/leadership role, or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to a personal vehicle for business-related purposes.



Skills and Abilities

  • Demonstrated knowledge of acute and chronic disease processes, medical, surgical, nursing and rehabilitation management.
  • Demonstrated knowledge of developmental disabilities, mental illness, behavioral challenges and their psychiatric and non-psychiatric management.
  • Knowledge of the principles of normalization and community living philosophy.
  • Demonstrated knowledge of a broad range of community programs, community resources and health related support services.
  • Demonstrated leadership and teaching skills.
  • Ability to deal effectively with others within an interdisciplinary team environment.
  • Ability to work independently and as a member of the a team.
  • Ability to organize and prioritize work.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to operate related equipment including applicable software applications.
  • Physical ability to carry out the duties of the position.
