Background: Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods; Flemingdon Park, Fairview and Thorncliffe Park. Position Summary:
The Community Health Registered Nurse, as part of the Primary Care team, is expected to work in an interdisciplinary team to deliver high-quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health Registered Nurse who is committed to client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care for the residents or Thorncliffe Park, Flemingdon Park and North York. Responsibilities include:
Direct Client Care:
- Provide comprehensive primary care within RN scope (expanded role).
- Coordinate care across the Health Centre and external providers.
- Provide telephone triage, advice, and follow-up on results and prevention needs.
- Deliver client education and health promotion during assessments and groups.
- Provide home visits and support for home-bound clients, as needed.
Health Promotion and Education:
- Work with internal teams to deliver targeted health education and support community development.
- Represent the Health Centre in community projects and partner activities, as needed.
- Support outreach program development based on community needs.
- Contribute to policies and strategies that advance public health and address determinants of health.
- Apply evidence-based practice aligned with evolving community needs.
Organizational Responsibilities:
- Participate in quality improvement, audits, accreditation, committees, and team meetings.
- Support annual planning and evaluation of program goals.
- Identify priority populations and help implement effective services.
- Maintain knowledge of community services and make appropriate referrals.
- Teach/supervise learners and support volunteer placements, as needed.
- Participate in professional development and staff in-service training.
- Perform other duties as assigned.
Skills and Qualifications:
- RN in good standing with the College of Nurses of Ontario (CNO)
- Bachelor of Science in Nursing (BScN)
- 3+ years RN experience in a Community Health Centre (preferred), including:
- Assessment and triage
- Chronic disease management (e.g., diabetes, hypertension, asthma)
- Medication/treatment administration (per medical directives)
- Symptom monitoring and clinician updates
- Women’s health screening and contraception counseling
- Travel health advice and pre-travel immunizations
- Pediatric and/or perinatal experience (asset)
- Strong assessment, health promotion/education, counseling, and community development skills
- Ability to work effectively on an interdisciplinary team
