$74,797.00 - $88,078.00 pro-rated based on FTE annually along with HOOPP pension plan and extended health benefits

Background: Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods; Flemingdon Park, Fairview and Thorncliffe Park. FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement. We are primarily funded through the Ontario Health.

Position Summary:

As a member of an interprofessional team, the Physiotherapist works within the framework of the social determinants of health to provide client-centered care that emphasizes health promotion and disease prevention. The Physiotherapist provides comprehensive assessment and treatment of neurological and musculoskeletal dysfunction. The Physiotherapist practices according to the standards of practice of the College of Physiotherapists of Ontario and to guidelines and policies established at the CHC where they are employed.

The Physiotherapist works in partnership with other staff and management to support the vision, mission and values of the CHC.

Physiotherapists will be expected to participate in and collaborate with a network of physiotherapists at different Community Health Centres in Toronto.

Duties & Responsibilities:
  • Provide assessment, diagnosis, treatment, health education and counselling for acute and chronic conditions (primarily an orthopedic caseload, but may include clients with COPD, diabetes, physical disabilities and other chronic conditions).
  • Develop rehabilitative group programs and services in more than one location.
  • Order and maintain an inventory of physiotherapy supplies and equipment
  • Refer clients to appropriate internal and/or external resources
  • Maintain complete and accurate electronic medical records
  • Participate in various quality improvement initiatives and interdisciplinary case conferences
Skills & Qualifications:
  • Graduate of an accredited school of Physiotherapy
  • Registration with the College of Physiotherapists of Ontario
  • Eligible for membership with the Canadian Physiotherapy Association
  • Up-to-date knowledge of physiotherapy theories, practices and procedures
  • 3 to 5 years of experience in a health care setting with preferred experience in the community, and a desire to work with low income families and multi-cultural communities
  • Excellent clinical, counselling and interpersonal skills
  • A commitment to best practices, ongoing learning, and client centered and collaborative care
  • Excellent computer and organizational skills
  • Second language relevant to catchment area populations is an asset

Working Conditions & Physical Requirements:
  • Incumbent will be expected to work some evening and/or weekend hours based on role demands.
