Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.

NYTHP is one of the initial 12 Ontario Health Teams chosen to accelerate their work to deliver home care in their local communities and also focus on seamlessly transitioning people experiencing chronic disease through their primary care, hospital and home and care needs.

Position Summary:

We are seeking an innovative Nurse Practitioner that will participate in leading the transformation of primary care across North York. In this role, you will drive change by integrating chronic disease prevention and management with a community-based, barrier-free approach to delivering essential healthcare services to local neighbourhoods.

  • You will co-design and deliver evidence-based care for complex chronic conditions such as COPD, CHF, and diabetes—emphasizing wraparound care, proactive transitions, and personalized supports.
  • Working with interdisciplinary teams, you will help reimagine primary care delivery by taking a comprehensive suite of preventive services directly to our communities. This includes robust cancer screening programs (cervical, breast, and colorectal), a full range of immunizations and vaccination services, routine health checkups, and additional preventive interventions.
  • These services will be delivered through mobile and pop-up clinics designed to respond to barriers to access, ensure timely follow-up, and bring effective care coordination. In this role, you will play a key part in reshaping primary care to enhance patient outcomes, engage underserved populations, and set new standards in access, equity and innovation.

Responsibilities Include:

  • Clinical Management & Integrated Care: Provide advanced, evidence‐based care for complex chronic conditions (e.g., COPD, CHF, diabetes) by performing comprehensive assessments, accurate diagnoses, and tailored treatments. This includes assessing upstream risk, prescribing medications (with advanced pharmacotherapy when indicated), ordering or recommending diagnostic tests, and managing smooth transitions for patients from hospital settings to community-based care as part of the NYTHP CDM Hub.

  • Patient Education & Quality Improvement: Empower patients and families through health coaching, motivational interviewing, and self-management strategies delivered via group sessions or one-on-one consultations. Contribute actively to the co-design and refinement of clinical workflows, standard operating procedures, and care pathways, while supporting ongoing outcome measurement and quality improvement initiatives aligned with the North York Toronto Health Partners framework.

  • Mobile/Pop-Up Preventive Care: Deliver barrier-free primary care through mobile or pop-up clinics by offering essential services such as Pap testing, comprehensive cancer screenings (cervical, breast, and colorectal), FIT testing, immunizations, and routine checkups.

  • Coordinated Follow-Up & Program Support: Ensure robust preventive care by actively following up on screening results, managing patients, especially those unattached—with abnormal findings until stabilized or integrated into ongoing care. Support CHIF initiatives by aligning new and existing programs with established clinical standards and contributing to the development of clinical guidelines and community-based education modules.

  • Interdisciplinary Collaboration: Work closely with physicians, nurses, social workers, community health workers, and other stakeholders to plan, implement, and evaluate comprehensive, patient-centred care.

  • Documentation & Compliance: Maintain accurate and timely client records in the EMR (e.g., Practice Solutions) in accordance with privacy legislation and College of Nurses of Ontario guidelines, while providing leadership in optimizing documentation processes.

  • Advocacy & Professional Development: Advocate for patients by aligning care with their health goals and social determinants, and actively participate in training, clinical engagement, and continuous professional development.

  • Quality & Strategic Involvement: Collaborate on quality improvement initiatives, audits, accreditation processes, and strategic planning committees to support FHC and NYTHP objectives.

  • Education & Community Engagement: Contribute to the training of students and colleagues, as needed, and build strong community partnerships through ongoing outreach and co-design efforts.

Qualifications, Skills, and Proficiencies:

  • Registered Nurse with the College of Nurses of Ontario in the Extended Class.
  • Baccalaureate in Nursing and graduate of recognized Educational Program for Primary Care Nurse Practitioners
  • 2-3 years of experience in primary care or community nursing will be an asset.
  • An ability to work as a member of an interdisciplinary health care team, responsible and accountable for own practice
  • Demonstrated experience in chronic disease management and prevention, and a good understanding of the role of health promotion
  • Experience working with marginalized communities and complex clients
  • Commitment to anti-racism and anti-oppression practices
  • Excellent oral and written communication skills
  • Ability to work flexible hours, including evenings and weekends
  • Ability to speak other languages, preferably Spanish, or Arabic is an asset
  • Knowledge of EMR System; an asset (Practice Solutions)
