SUMMARY OF THE POSITION
Person who participate in the identification of the needs of the population, participate in the community organization & development and execute different action & programs by using appropriate techniques, may act as a resource person for the groups.
While creating, developing and nurturing culturally appropriate interactions and connections with each other, clients and the community, the person provides professional expertise and consultation to the primary health care team.

SPECIFIC FUNCTIONS
The Community Worker, as part of the Miyupimaatisiun Care Team, plays a key role in promoting social well-being for individuals, families, and communities. They assess needs, mobilize resources, and collaborate with professionals to enhance clients’ social and psychological functioning. Through culturally appropriate interactions, they provide expertise and consultation to the primary health care team.

• Providing clients within their pods with a stable and ongoing relationship and offering guidance through various services as required;
• Develop a helping relationship with their clients, using a trauma-informed approach, creating a safe and supportive space for disclosure;
• Provide short-term and solutions-focused interventions, such as crisis management, and assistance with situations that can be resolved in a few sessions;
• Remains available to address the spontaneous needs of clients followed by their primary care team;
• Engage in case discussions with other professionals within the primary care team and extended team;
• Promote and engage client’s participation and their network in decision-making, conduct family group conferencing to increase the commitment of the family;
• Identify priority needs of individuals, groups and families;
• If a long-term follow-up or specialized services is required, make the referral for the client to access services;
• Maintain accurate documentation of the information collected using MYLE (progress notes, reports, forms, etc.);
• Establish connections with the psychosocial extended team, team leaders or with appropriate resources (team meetings, case discussions, etc.);
• Use a set of principles and guidelines to ensure a professional practice that prioritizes the well-being of clients (confidentiality, policies, clinical process, etc.);
• Participate in an ongoing process of self-awareness, learning and growth to enhance one’s professional competencies.
REQUIREMENTS
Education:
• Must have a college diploma (DEC) in social work techniques or another appropriate college discipline from a school recognized by the relevant ministry is required.
• This position also includes individuals holding a college certificate in social work techniques or social assistance techniques
• Completed or committed to achieving the following training: Nitutaamh, DPSQA Psychosocial onboarding, Suicide Risk assessment, and any other relevant training identified by management.
Knowledge and Abilities:
• Strong knowledge of Cree culture, social and health issues in Eeyou Istchee.
• Familiarity with Eenou/Eeyou Pimaatsiiun healing practices and paradigms.
• Skilled in building helping relationships through active listening and effective communication.
• Ability to conduct interviews, clarify information, and identify risk/protective factors.
• Proficient in writing progress notes and reports using computer systems.
• Experienced in working with professionals, community members, and multidisciplinary teams.
• Understanding of Youth Protection, Young Offender procedures, and relevant laws (Y.O.A, Y.P.A, L.H.S.S.S).
• Strong counseling skills; sensitive to family dynamics and confidentiality issues.
• Demonstrates initiative, dynamism, responsibility, and professionalism.
Language:
• Fluent in Cree and English;
• Fluency in French is an asset;

Other:
• Must be ready to work occasionally outside the regular working hours (availability);
• This position includes on-call periods.
