Description
Located in N.E. Calgary, the Peter Lougheed Centre (PLC) is an acute care hospital offering both inpatient and outpatient services. The PLC has over 600 inpatient beds, including Medical, Surgical, Pediatrics, Women’s Health and Critical Care. The PLC is the Vascular Centre for Southern Alberta, Southwestern Saskatchewan and Southeastern British Columbia. Under general supervision from the Social Work Manager, the successful candidate will be expected to provide Social Work services primarily to the Emergency department and as needed to provide coverage support to: medical/surgical units, maternity/pediatrics units, critical care, and outpatient clinic areas at the PLC. These services are primarily clinical in nature, and include advanced level assessment and treatment (including, but not limited to: psychosocial assessments, counseling, case management and discharge planning). This caseload may change based on operational needs. This position requires the ability to work independently in a fast-paced environment and yet function as part of the interdisciplinary team. At all times the Social Worker ensures he/she is compliant with ACSW standards of practice.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Relevant experience in an adult acute care setting in last 3 years. Relevant experience in the Emergency Department in last 5 years. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification required. Excellent interpersonal and communication skills to interact with members of the interdisciplinary team, patients and families in a dynamic health care setting. Demonstrated problem solving, critical thinking and conflict resolution skills. Excellent organizational and time management skills to efficiently organize and prioritize workload to keep pace in a busy multi-disciplinary patient care. Recent experience in crisis intervention and working with patients/families facing chronic and/or life threatening illness required. Experience and advanced skill in providing therapy to individuals and families. Demonstrates behaviours consistent with the values of AHS - Compassion, Accountability, Respect, Excellence, and Safety.
