Within Cancer Care Alberta (CCA), Supportive Care services are provided by a team of health care professionals dedicated to enhancing the quality of life of those affected by cancer in Alberta, by providing support from the time of cancer diagnosis, through treatment and beyond. The team of professionals that work within CCA are leaders in cancer prevention, diagnosis, treatment, survivorship, and palliative care, all on a foundation of world-class research. We are seeking a temporary Social Worker (SW II) passionate about cancer care to join our team at the Central Alberta Cancer Centre. You will support patients from Red Deer as well as surrounding communities in Northern Alberta. As a SW II, you will play a vital role in empowering patients experiencing cancer to address the social determinants of health. Key Responsibilities include: Referral triage, screening/intake, resource counseling, supportive counseling. Working collaboratively with interdisciplinary team of healthcare professionals to enhance the patient experience. Developing and facilitating psychoeducational groups. Participating in quality improvement initiatives, research, and service evaluation. Engaging in continuous professional development and education. Supporting orientation and mentorship of new employees and students. Contributing to a collaborative team environment with excellent time management and communication skills. Connecting patients to non-AHS programs such as Wellspring Alberta, the Canadian Cancer Society and other community programs. The caseload for all positions within Psychosocial Oncology may be subject to change based on operational considerations and will include virtual as well as in-person supports.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Basic Heart & Stroke Foundation Cardiac Life Support-Healthcare Provider (BCLS-HCP). A minimum of 2 years post-degree medical (Social Work) experience. Experience working in an urban adult cancer care setting, palliative care, inpatient and/or outpatient care in the last 2 years. Experience providing virtual care within the last two years. In-depth knowledge and experience of advance care planning, income security programs, housing, and support services. Demonstrated skills in assessment, intervention, system navigation and case management. Professional development related to psychosocial oncology within last 2 years. Proficient with Connect Care/EPIC. Proficient with Microsoft Office Suite. Experience providing supportive counseling. Ability to work in a busy healthcare environment with competing priorities and ability to effectively manage time.
Experience working with chronic disease in adult urban outpatient setting within last 5 years. Experience in adult acute care within last 5 years. SW Connect Care training. Experience facilitating groups & providing education to patients/families about cancer. Experience and knowledge of community and financial resources, grief & loss. Experience developing partnerships/collaborations within and across organizations. Experience with the Patient Financial Assistance Program.
