Foothills Medical Centre (FMC) is an Acute Care hospital with over 1,100 beds. It is the Trauma Centre for Southern Alberta, Eastern British Columbia and Western Saskatchewan. FMC has a Neuro Tertiary Unit as well as the majority of In-Patient Neurological services in Calgary. The present caseload for this position is in OPTIMUS (MS rehab) with the potential for coverage on any unit/service as required. The successful incumbent will show proficiencies in the following areas: Psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counseling, case management and coordination. Interpersonal skills with the ability to maintain collaborative working relationships with members of the interdisciplinary team. Self-direction and independent decision making, sound clinical judgment and critical thinking skills. Time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic and challenging environment. Effective communication with a patient and family centered focus. Demonstrates problem solving, critical thinking and conflict resolution skills. Creating innovative solutions for complex and diverse issues. Demonstrates commitment to and participation in activities which promote professional and personal growth and development. Actively integrates patient and family centered care principles into own clinical practice. Demonstrates behaviors consistent with the values of AHS - compassion, accountability, respect, excellence and safety.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Experience within the past year working in the area of adult acute care including multidisciplinary team settings. Proven experience in providing counselling, therapy, and support services to address various issues, such as mental health concerns, relationship difficulties, and access to resources. Experience in group facilitation. BLS for Health Care Providers Level C CPR certification issued within the last 12 months prior to commencement of employment.

Minimum five years post qualifying experience in an acute care or health care setting. Experience within the past three years working in a fast-paced multidisciplinary setting. Experience working with chronic disease patient populations.
