Social Worker II

This temporary position will be providing Social Work support to acute care units of Chinook Regional Hospital in Lethbridge. Services include providing support counselling, resource support and advocacy. You will participate in discharge planning and working within a multi-disciplinary environment. Awareness of community resources including housing, finance addiction, mental health, crisis, grief counselling are essential in this role.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

As required.

Minimum of 2 years of social work experience preferred. Experience working in health care setting an asset. Knowledge of Epic/Connect care.
