Description
This Social Worker II position provides services to clients and families who receive homecare services as well as acute care services in Cardston and Fort Macleod. The Social Worker II provides bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive counselling, advocacy and consultation as part of their day-to-day work. The Social Worker II has excellent working knowledge of community financial resources and medical coverage for adults and families. The Social Worker makes appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of clients and families. The Social Worker II contributes to non-patient care activities including program planning, evaluation, quality improvement research and staff development, along with other duties as required by the position.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Minimum 2 years of professional Social Work experience. This position requires a high degree of independence, critical thinking, and problem-solving skills, an ability to prioritize workload. Heart & Stroke Foundation - Basic Life Support certification.
Knowledge and ability to use computer applications such as Microsoft Office, Outlook and Teams. Previous experience working in the health care system and EMR such as Connect Care. Excellent written and verbal communication skills.
