The city also has a vibrant arts and culture community with an art gallery, a few museums, and live theatre. There are also a multitude of dining and shopping opportunities. Alberta Health Services is invested in Grande Prairie with opening the new Grande Prairie Regional Hospital. As one of our Social Work team, you will be based in the Grande Prairie Virene Building or in Beaverlodge Hospital and provide services to the Beaverlodge and Grande Cache areas in close collaboration with Acute Care and Continuing Care, which provides services to Home Care, Designated Supported Living, Lodge and Long Term Care. These services are primarily clinical in nature and require expertise in financial and resource management, psychosocial assessments, counseling, case management and discharge planning. This also involves consultation and collaboration with medical staff and community agencies. This position requires the ability to work independently with occasional review, consultation and evaluation from a Manager or Coordinator and the ability to work as part of an inter-disciplinary team. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Current BCLS-HCP certification required. Ability to organize and set priorities. Considerable initiative and judgment required. Experience working on an inter-disciplinary team required. Good computer skills are required. This position requires a vehicle for business use. Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel all year round is required.
Medical experience with acute care, home care, Type A and Type B facilities is preferred. Connect Care training is preferred.
