The Children’s Rehabilitation Services program has an exciting opportunity for a Social Worker to serve as a Community Services Coordinator on our team! You will support program areas providing social work support to clients and their families, along with rehabilitation staff in Children’s Rehabilitation Services. You will work collaboratively with a multi-disciplinary team providing support and case coordination to address the health needs of clients and families. In this position, you will receive referrals from community partners (including families, public health, and physicians) and act as liaison with those partners. You will complete psychosocial assessments to determine client need and match those needs with appropriate internal or external services/resources. Your proficiency in psychosocial assessment, demonstrated familiarity with community resources, awareness of health and disability support services and familiarity with the Health Information Act and privacy legislation will be essential for your success in this position. Strong interpersonal/teamwork skills and the ability to organize and prioritize are essential. This position includes an enhanced role supporting and coordinating services for indigenous children, families and communities within Central Alberta. Some of the roles for this position include: Helping the team to provide holistic care for Indigenous children and youth and their families Educating health care providers about respectful relationships and health care for Indigenous people and building understanding and trust between Indigenous people and health care providers Acting as a liaison and key contact with Indigenous communities, First Nations leadership, Métis Nation and Métis settlements and other stakeholders Join a team of skilled and dedicated health care professionals - we are passionate about what we do!

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Possession of a reliable vehicle for business use and work-related travel. Traditional Indigenous knowledge and experience (cultural preparation is based in oral cultures and not measured by diplomas and degrees) Traditionally earned understanding about Indigenous cultural beliefs, values, customs, traditions, healing ways, ceremonies, traditional medicine, protocols. Guided ongoing development with traditional Elders over the course of many years. Learning about Indigenous cultural identity that is recognized by other indigenous people and is respectful in a health care environment. Knowledge of how to respond to diverse cultural needs of a wide variety of Indigenous people. Knowledge about the effects of colonization and residential schools, including intergenerational trauma. Openness to continually learning. Experience and understanding of indigenous children & youth ages 0 to 18 years and ability to support and guide children, youth, parents, guardians, families, providers and community partners through the health services journey in a respectful and knowledgeable way.

Experience in pediatric rehabilitation. Experience in Education, Health and Children’s Services. Minimum of 2 years experience working in a Social Work position. Proven ability to exercise professional judgement, use initiative, demonstrate strong organizational skills and effectively participate as a member of a multidisciplinary team. Proficiency in Microsoft Word, Power Point, Outlook Express and the Internet.
