Reporting to the manager of the Acute Care Addiction Recovery Program (ACARP), the Social Worker II - ARCH works as a key member of the multidisciplinary team. The SW collaborates with interdisciplinary team members to deliver patient-centered, trauma-informed approach to care practicing within a harm reduction philosophy. The goal of the ARCH team is to provide patient-centered, evidence-based, holistic care to patients who are socially vulnerable and/or dealing with problematic substance use. By using a coordinated, multidisciplinary approach, the team strives to support at risk patients improve their social determinants of health. The SW role includes conducting comprehensive assessments and interventions that address the social determinants of health. The SW role also includes delivery of sterile supplies, patient education, overdose response kit distribution, and other education and support. The SW is a key member of the multidisciplinary team and participates in complex discharge planning for patients with substance use disorders and/or social inequities. The SW role primarily works out of the ARCH transition clinic.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Experience working with inner city/vulnerable populations. Demonstrated experience of working within a harm reduction framework. Demonstrated proficiency in the assessment of patients with substance use disorder. Knowledge of screening tools used for assessment of substance use and treatments. Demonstrated ability to support patient’s transition to community resources to ensure continuity of care. Strong communication skills. Ability to work within a multidisciplinary team.
