Description
The Family Care Clinic is known for its innovative approach to providing comprehensive primary care for people with complex needs. This could be any combination of chronic illnesses, mental health difficulties and addiction along with social, economic or cultural factors that negatively impact health. The Clinic is located in the East Calgary Health Centre. Reporting to the ECFCC management and under supervision of psychologist II, the Social Worker II will demonstrate commitment to collaborative practice to enhance patient health outcomes; ability to work independently within a complex and rapidly changing environment; respect for diversity using a supportive, non-judgmental approach; conflict resolution, problem solving, critical thinking and leadership skills are all essential to succeed. This position has minimal Saturday shifts in rotation.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Minimum of 2 years experience working as a registered Social Worker. Minimum of 5 years recent experience working with at risk families providing interventions and support around intrinsic needs. Experience working in a community setting with vulnerable populations. Demonstrated ability to work with individuals from diverse backgrounds and cultures using a supportive, non-judgemental approach. Experience in working from a trauma informed lens. Demonstrated ability to collaborate effectively with various community partners. Demonstrated ability to function in a collaborative interdisciplinary environment. Demonstrated ability to use Electronic Medical Record (EMR) and overall computer literacy required
Experience working in a primary care setting. Experience working with interdisciplinary team. Experience with adult education and group facilitation. Demonstrated flexibility and adaptability within a complex and rapidly changing environment and may involve working with clients who present with criminal or suicidal tendencies; potential exposure to violence and aggression.
