Social Worker II

The Social Worker II will deliver primary support to the ACH acute care inpatient units and will be responsible for providing: intake, psychosocial assessment and support; specialized resource intervention; risk screening, group facilitation; family education, supportive counselling, discharge planning and consultation to other health care providers. Working closely with the Social Worker III role within this clinical area, the Social Worker II will support collaboration within the inter-disciplinary psychosocial, rehab and medical and surgical teams and a range of service providers to facilitate positive patient outcomes. As the successful candidate, you will demonstrate the ability to work collaboratively within an interdisciplinary team; will exhibit excellent clinical reasoning & problem-solving skills; will be organized in managing a diverse & complex caseload; will demonstrate initiative, adaptability & flexibility to meet varying clinical & operational needs; will demonstrate effective interpersonal & verbal/written communication skills. Supervision of students is required. Participation on program/clinic committees is expected. You will receive guidance, mentoring, & leadership appropriate to your level of experience.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Recent and a minimum 3-5 years’ experience in acute pediatric care (experience including within the last year). Training and ability to deliver psychosocial interventions from a trauma-informed lens, in the context of crisis and chronic illness management models. Knowledge of child development. Education and experience supporting patients and families with adjustment to diagnosis, medical compliance and grief and loss. Experience working with the intersection of child welfare and medical Social Work to address child maltreatment and neglect, and domestic violence. Experience working with community resources and early intervention programs supporting special needs populations. Experience working on an inter-disciplinary team. Experience managing referrals from multiple units/programs.
