Description
Operating out of seven community sites across the city of Calgary and surrounding areas of Cochrane, Airdrie, and Chestermere, the Pediatric Community Rehabilitation Program provides services to children and youth that align with provincial health priorities. The Pediatric Community Rehabilitation (PCR) social worker provides a range of generalist Social Work services to clients and their families within established standards, directives and policies to enhance child and family participation in rehabilitation services. The PCR social worker provides a range of services including psychosocial assessment, resource-based interventions, intake and risk screening, counseling, case management and coordination, transition planning, prevention and promotion and public awareness activities and other duties as assigned. The position also involves consultation and collaboration with interprofessional team members and community agencies and requires the ability to work independently with occasional review, consultation and evaluation from a manager. The position requires the ability to work independently with a high degree of judgement and must be able to work effectively as part of a multidisciplinary team and commit to strengthening team dynamics by actively facilitating multidisciplinary collaboration to enhance client outcomes. PCR Social Workers participate in peer mentoring, student placements, contribute to quality improvement and their professional community. This is a community-based program and travel between sites may be required. Our therapists are involved in a variety of programs and evening work may be required.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
As the successful candidate, you will demonstrate proficiency in the following areas: Experience working in a community based pediatric rehabilitation setting. Interpersonal skills with the ability to develop and maintain collaborative working relationships with members of the multidisciplinary team including families. Self-direction, independent problem solving and decision-making, sound clinical judgment and critical thinking skills. Time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic, fast paced, multidisciplinary environment. Actively integrate patient and family centered care principles in all areas of practice including clinical decision-making and effective communication. Create innovative solutions for complex and diverse issues. Demonstrate behaviors consistent with the Values of AHS: Compassion, Accountability, Respect, Excellence, and Safety. Practice according to clinical practice standards. 2-5 years' experience working with a pediatric population and children with complex needs.
Experience supervising students. Experience in a pediatric community setting Experience in Program Development. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
