Description
The Pharmacist I will play key role in providing quality healthcare and will have opportunity to learn and grow within an inter-professional team supporting Alberta Kidney Care North. T he ambulatory role will provide care to patients undergoing hemodialysis at the in-center dialysis unit located at the University of Alberta Hospital. The pharmacist will perform comprehensive medication reviews to identify, resolve and prevent medication-related problems. This is accomplished through collaborating with members of the health care team including the patient and/or their caregivers, to formulate a medication treatment plan and work to optimize patient outcomes. Duties include selecting, initiating, modifying, or administering medication therapy, monitoring the patientʼs response to therapy, and documenting all direct patient care activities. Provision of education to patients as well as coordination and integration of medication therapy management services is also an expectation. Virtual support for dialysis patients in other in-center clinics may also be required. The pharmacist is responsible for providing drug information, education and potential research activities for the Alberta Kidney Care North program. Mentorship and student precepting will be an expectation.
As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.
Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).
Additional Prescribing Authorization (APA) Recent direct patient care experience and demonstrated ability to work to full scope of practice including but not limited to, APA, laboratory ordering. The candidate must be self-directed, well organized and capable of making decisions while working independently. Candidates must demonstrate excellent communication and interpersonal skills and have a clear focus on self-directed learning and continuing education.
Completion of a Canadian accredited pharmacy residency program or a post-graduate clinical pharmacy degree, e.g. PharmD. Active membership with the Canadian Society of Hospital Pharmacists (CSHP). Pharmacy related experience in a hospital setting or in direct patient care providing comprehensive medication management services. Previous experience in nephrology, transplant of internal medicine areas. Minimum 2 years Clinical Pharmacist experience within the last three years.
