Pharmacist I - Home Parenteral Therapy

Are you a passionate, driven Pharmacist ready to make a meaningful impact on patient care? Alberta Health Services invites motivated professionals to join our dynamic Home Parenteral Therapy (HPT) Program, where your expertise will make a difference every day. Working collaboratively within an interdisciplinary care team, you will help ensure medications are used safely, effectively, and thoughtfully to achieve optimal patient outcomes. Based out of the Royal Alexandra Hospital, University of Alberta Hospital, and Kaye Edmonton Clinic, you will support the HPT program through daily huddles, clinical collaboration, medication counselling, and comprehensive documentation to ensure seamless, high quality care across care settings. You will play a key role in identifying and resolving medication related issues, offer meaningful patient and family engagement through developing and implementing individualized medication management plans, and continuously optimizing therapy through ongoing assessment and teamwork. Responsibilities include assessing and verifying medication orders and supporting the preparation and verification of parenteral therapies. As the successful candidate, you will demonstrate a commitment to self directed learning and ongoing professional development, excellent communication and interpersonal skills, strong organizational and problem solving abilities, and the capacity to work independently while thriving in a collaborative team environment. Strong time management, adaptability, and self motivation are essential. You will be a preceptor and mentor to students and residents and other staff.

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Completion of Bachelor of Science in Pharmacy. Current, provisional or transferable registration with the Alberta College of Pharmacy (ACP) as a Pharmacist.

Recent direct patient care experience (at least 1 year within the last 3 years) as part of a multidisciplinary team. Additional Prescribing Authorization (APA) is required. Fluency in the English language (both written and verbal). To ensure your skills and qualifications are considered, please attach a resume and cover letter with your application.

Completion of an accredited hospital residency or post-baccalaureate Pharm D program. Experience within HPT. Experience in an ambulatory clinic setting. Experience with infectious disease management and IV therapy. Experience with Connect Care. Community experience and understanding of drug coverage plans and Kroll. Active membership in the Canadian Society of Hospital Pharmacists (CSHP).
