Step into a pivotal leadership role that shapes how patients transition safely and seamlessly from acute care to community living across the Central Zone. As the Manager, Transition Services, you will lead a dynamic, multidisciplinary team dedicated to improving patient flow, enhancing care coordination, and elevating the client and family experience. In this role, you will guide Transition Coordinators and specialized clinical teams working across emergency settings, acute care units, and community-based services. You’ll drive innovative practices, champion quality improvement, and support the integration of services across Acute Care, Primary Care, Community programs, and Assisted Living Alberta. You will collaborate closely with clinical leaders, physicians, community partners, and senior leadership to strengthen relationships, enhance system performance, and advance strategic priorities. The Manager, Transition Services, provides strategic and operational leadership to clinical teams supporting patient flow and care transitions across the Central Zone. This role leads multidisciplinary teams who coordinate safe, person‑centered transitions from emergency and acute care settings into community programs-ensuring timely access to appropriate services, supporting continuity of care, and improving overall system flow. Reporting to the Transition Services Director, the Manager champions innovation, quality improvement, and integration across Acute Care, Primary Care, Community programs, and Assisted Living Alberta. The role oversees Transition Coordinators and specialized assessment teams, supports process redesign, monitors performance indicators, and fosters collaborative relationships with site leadership, physicians, partner programs, and community agencies. This position plays a key role in operational planning, staff development, workload management, and change leadership. The Manager ensures services align with relevant standards, legislation, and provincial policies while cultivating a culture of safety, accountability, and continuous learning. The role also supports resolution of complex client and family concerns, contributes to program planning and evaluation, and maintains effective communication across diverse stakeholder groups. Baccalaureate Degree in Nursing and active registration with the College of Registered Nurses of Alberta (CRNA). Minimum 10 years of experience in healthcare, including at least 5 years of clinical practice in Adult Home Care. Minimum 3 years of leadership or management experience in a health-related field, ideally within AHS/ALA. Demonstrated leadership in multidisciplinary teams with strong skills in communication, coaching, conflict resolution, and decision making. Competence with Continuing Care assessments (inter-RAI HC/CA) and Connect Care (Acute & Home Care). Proficiency with Microsoft Teams, Outlook, Word, and Excel. Valid driver’s license and reliable vehicle. Proven ability to manage complex service delivery environments, lead change, and support quality improvement initiatives. As Required.

Master’s degree in a relevant field. More than 7 years of management experience. Strong analytical and problem solving skills. Experience in Acute Care and/or Community services, with current Central Zone experience considered an asset.
