If you enjoy a role in providing services to clients in a community-based setting, then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse - Case Home Care Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. Possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting up to 30 libs lowering, carrying, pushing and pulling). This position is based out of Didsbury and provides care services to Didsbury, Carstairs, Crossfield and surrounding rural areas.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

As required.

Completion of a post-basic Medication Administration Course for LPNs. Recent work experience in a community-based setting, Home Care experience preferred. Palliative care experience. Advanced wound care knowledge & experience. Computer and technology skills including a good understanding of Email/Outlook, Excel and iPhone.
