The Licensed Practical Nurse- Inpatient Transition Services Coordinator (TSC) screens and assesses the needs of individuals and their families who cannot function independently in the community. Functioning in a case coordinator role within the multidisciplinary environment, the TSC assesses plans, implements interventions, and evaluates the overall plan of care for clients who may present at any point of their lifespan with varying health needs. This position entails coordinating a broad range of service and community resources. The role also involves maintaining close communication with families, attending physicians and other health providers in acute or long-term care or community settings. Provides ongoing guidance and mentoring to acute care unit staff to ensure the homecare referral processes are followed and ensures order are appropriately written for successful community care. Provides clients/families with information and counseling with respect to care options and facilitates access to these. The TSC establishes and maintains effective communication and liaise with physicians and others involved in the clients care. Presents clients situations that exceed available resources to management to assists in the complex placement of unique clients. Supports client to successfully transition across care setting. Develops and maintains relationships with Home Care Case Managers, Integrated and Supportive and Facility Living Case Managers, Facility leadership, and other agencies. The TSC negotiates and advocates for the needs of the clients to find a suitable and successful placement. The successful candidate will be expected to cover and have an understanding of all caseloads and TS processes.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Completion of an accredited practical nurse education program. Active, or eligible for, registration and a practice permit as a Licensed Practical Nurse with the College of Licensed Practical Nurses and Health Care Aides of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

1 year (in the last 3 years) community experience (TS/homecare/ISFL) Physically able to walk 3-5 km daily; carry/lift/pull resource materials weighing up to 25 lbs; be on feet long periods; provide physical assistance to clients if intervention for safety required; manage repetitive adjustment and exertion, including sitting/standing/bending Ability to work with a diverse client population, including clients with complex medical, mental health, and behavioral care needs, challenging behaviors, social situations/dynamics. Ability to work in a fast-paced environment and manage stressful situations. Excellent organizational and time management skills to efficiently organize and prioritize workload Demonstrated ability to provide coverage in all TS caseloads, flexibility with assignment/caseload changes as indicated by operational needs Perform up to 6 hours up keyboarding (min 40 wpm) and written documentation per shift while navigating a variety of IT programs.

Prefer recent experience in Transition Services, ISFL and/or Home Care Knowledge of some Community resources
