Description
The SHC has four key pillars of service: Collaborative Practice, Wellness, Patient and Family Centered Care, and Innovation. These pillars guide the development and delivery of service to all patients and families at SHC. The Home Parenteral Therapy Program (HPTP) Licensed Practical Nurse (LPN) works as a member of the HPTP team to meet the needs of outpatients requiring parenteral therapies. The LPN provides nursing assessment, planning, implementation and evaluation of care for the individual requiring Home Parenteral Therapy. This position will appeal to an LPN who values working in an autonomous and collaborative role within a multidisciplinary team. The patient population spans a broad diagnostic range and therefore is well served by the LPN who functions at a high level of critical judgment and clinical decision-making. The HPTP team comprises Licensed Practical Nurses, Pharmacists, Infectious Diseases Physicians as well as other support staff. HPTP staff work in close partnership with Home Care Nursing. The Medical Ambulatory department within SHC is currently recruiting a permanent full time Licensed Practical Nurse (LPN). Reporting to the Unit Manager of medical ambulatory services, the LPN is responsible for the coordination and care of patients in the Home Parenteral Therapy Program (HPTP) at SHC. The HPTP team assists in providing service and care to patients for Infectious Diseases Physicians providing urgent care for patients referred from the ED or Inpatients. The patient population spans a broad diagnostic range and therefore is well served by the LPN who functions at an elevated level of critical judgment and clinical decision-making. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Previous experience in an outpatient clinic (minimum one years’ experience in the last three years in an FTE). Three years of current acute care medical / surgical clinical experience (within the last 5 years). Aptitude for or demonstrated excellence in in initiating and maintaining, and in decision-making related to venous access. Aptitude for or experience in adult teaching and learning required. Ability to function effectively in a fast-paced changing environment required. Ability to work independently as well as within a team environment required. Must be willing to take direction from physicians and other Registered Nurses/LPNs. Strong critical thinking skills within scope of practice, including clinical judgment, and problem-solving skills required. Must be self-directed, organized, motivated and able to prioritize and keep pace in a busy clinic environment. Excellent English verbal and written skills required. Excellent interpersonal and communication skills required. Excellent time management skills required. Must be able to assist patients with mobility deficits; ability to transfer patients to stretcher required. Must be able to work in a physically challenging environment: constant walking, lifting, standing and bending is necessary. This position requires on-call duties as per the rotation.
Previous experience in HPTP preferred. Interest in and expertise in comprehensive patient assessments and patient education preferred. Current expertise in management of parenteral therapies preferred.
