If you enjoy a role in providing services to clients in a community-based setting, then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Case Home Care Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. Possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting up to 30 lbs lowering, carrying, pushing and pulling). This position is based out of Didsbury and provides care services to Didsbury, Carstairs, Cremona, Crossfield and surrounding rural areas.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Knowledge, skill, ability and willingness to work to full scope of practice. Commitment to continuous learning relevant to LPN practice Ability to utilize critical thinking skills in the practice of conducting initial and ongoing nursing assessments, care planning and service delivery. Ability to utilize critical inquiry to assess, plan and evaluate the effectiveness of interventions on client outcomes. Excellent verbal and written communication skills Valid drivers’ license and access to a vehicle in good repair for work related purposes

Completion of a post-basic Medication Administration Course for LPN’s. Recent work experience in a community-based setting, Home Care experience preferred. Palliative care experience. Basic use of technology and competency in computer, clinical electronic device use, and electronic documentation. Recent experience working with complex wounds, surgical drains, & IV (HPTP) therapy
