Acting as an advocate, coordinator and liaison for the patient, the LPN identifies needs and sets priorities in collaboration with the multi-disciplinary team. The LPN participates in nursing assessment/interventions, care planning, implementing prescribed treatments, administering medications, evaluating treatment, and monitoring/documenting treatment progress and patient response. The LPN will also participate in quality monitoring activities and facilitates patient and family centered education and discharge planning in accordance with the established standards, and the vision, values, policies and procedures of Alberta Health Services. The LPN will work under the supervision of Registered Nurses and practice according to the policy and procedures set out by AHS and maintain competent practice and provides care according to CLPNA regulations. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Successful completion of CLPNA modules - Standards of Practice on Restricted Activities and Advanced Practice as well as certifications in required competencies. Basic keyboarding/computer skills required. Excellent communication skills required. The ability to balance numerous demands and conflicting priorities is essential. The candidate must be able to work well independently and within a team. Demonstrates critical thinking skills within scope of practice, including clinical judgment, problem solving skills and strong decision-making ability. The successful incumbent will be capable of performing lifts, transfers, repositioning, and other physically demanding activities associated with this role, of at least 40 lbs regularly.
Experience working within the CoACT framework and experience with Connect Care is preferred. It’s Your Move training is an asset. IV Therapy and PICC certifications are preferred.
