The Licensed Practical Nurse (LPN) functions as a member of the AHS Addiction and Mental Health team, Community In-reach, providing direct client care in the community shelter and is an essential member of an inter-disciplinary, recovery orientated, health care treatment team. The LPN will provide direct nursing care and support in the shelter program that serves the homeless population within our city. Collaborating with other AHS teams and with shelter staff, the LPN will provide cohesive, client-centered care, focusing on a holistic approach. The LPN will have a strong functional knowledge of the effects of substances including opioids, stimulants and alcohol and community resources to support recovery. The LPN will build and sustain strong collaborative relationships with community partners and will complete referrals for recovery-oriented systems of care. This unique position will allow the LPN, in consultation with the manager, to create programming that will meet the complex needs of the shelter clients while also integrating with the shelter team. The LPN will use a trauma informed lens to provide non-judgmental care in the shelter setting. The LPN will provide client case management, complete wound care, administer medications as ordered, respond to suspected opioid poisonings and provide emergency nursing response to client needs in the shelter setting. The LPN will work as part of the AHS Community In-Reach team to ensure optimal client care is provided in the shelter setting.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

3-5 years of experience working in Addictions and Mental Health, experience working with vulnerable populations. Physical ability to rapidly respond to emergencies in and around shelter setting including navigation of stairs.

Wound care experience, case management experience, working knowledge of substances and working with clients with complex needs.
