As the Licensed Practical Nurse covering the County of Flagstaff, you will have the opportunity to make a difference in the lives of people in your community. Reporting to the Care Manager, you will use your strong assessment, organizational, and leadership skills to deliver quality nursing care to clients in the community and to provide leadership and supervision of Health Care Aides. The successful candidate will play a key role on the interdisciplinary team, collaborating in the delivery of holistic, client and family centered care. Your role will require you to work independently in client homes, lift and carry medical bags and be comfortable working in all weather conditions. In this role, you will utilize your strong oral and written communication, and deliver quality nursing care to clients in all developmental stages in the community. Utilizing your experience in wound care, palliative care and client teaching, you will take a collaborative approach to meeting health goals, contribute to the nursing diagnosis and participate in nursing interventions and outcomes. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Must be able to meet the physical demands of the position: This position requires frequent physical demands including walking, bending, lifting, and standing and driving for long periods of time. Transferring patients of varying sizes and degrees of mobility. Transporting supplies and equipment. Driving in all weather conditions.

Appropriate competency in Meditech, Care Manager and RAI-HC programs and knowledge of Outlook and MS Teams. Broad knowledge of provincial, regional, and local community resources. Minimum 1 year previous Home Care and/or geriatric experience.
