Sedgewick Community Health Centre is looking to welcome a new Home Care Aide to its Home Care Team supporting the Flagstaff County Area. As a Home Care Aide and team member, you will demonstrate genuine interest and respect for our clients and utilize your knowledge, skills, and abilities to provide direct client centered care. Within the Home Care Interdisciplinary Team, the Home Care Aide is responsible for providing personal care and assisting with activities of daily living within the client’s authorized plan of care. The shifts for this position are 6.5 -7.75 hours, days, evenings and weekends. Flexibility is required as this is shift work. As a Health Care Aide (HCA), you will work as a collaborative member of the multidisciplinary team, providing personal assistance by supporting activities of daily living and delivering support services to patients (clients, residents) who require short term assistance or ongoing support, in accordance with the patient care plan and facility policies and procedures. In this role, you will work closely with patients, families, and caregivers across the continuum of care in home, community, and health care facilities. You will assist nurses with the provision of routine care to meet patients' hygiene, nutrition, mobility, recreation, and safety needs; assist with organized activities or programs; and observe and report on patients' mood, health, and wellbeing. Working under the supervision of a regulated health professional, you will assist in maintaining a safe environment for patients, staff, and visitors, while supporting effective communication with patients and health care professionals.
Active or eligible for registration or practice permit with the College of LPNs and HCAs of Alberta (CLHA) as a Health Care Aide, along with proof of education or completion of Health Care Aide Certificate within 18 months of hire.
Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) with Heart and Stroke Foundation. Valid Class 5 Driver’s License (GDL license is not acceptable). Must be able to meet the physical demands of the position: This position requires frequent physical demands including walking, bending, lifting, and standing and driving for long periods of time. Transferring patients of varying sizes and degrees of mobility. Transporting supplies and equipment. Driving in all weather conditions. Basic computer skills knowledge of Outlook and MS Teams.
Medication Assistance Certification and experience. Connect Care training/experience. Minimum 1 year experience within the last 5 years in a Home Care environment.
