Community Health Registered Nurse, Health Access Thorncliffe Park (HATP)

JOB DESCRIPTION

Title: Community Health Registered Nurse, Health Access Thorncliffe Park (HATP)

Employment Terms: 1 year Contract-Pregnancy/Parental leave (35 hours/week, with flexibility to work evenings and weekends)

Salary Range: $59,120 - $69,970 annually plus 4% vacation Pay

Expected Start Date: September 2019

Reporting to: Manager, Primary Health Care, HATP

Location: Health Access Thorncliffe Park (HATP) sites. This position will be located within any of HATP service locations based on community and program requirements.

Background: Heath Access Thorncliffe Park (HATP) is committed to providing coordinated Health & Wellness services to the Thorncliffe Park community. It is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the TC-LHIN. HATP is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park.

Note: This is a HATP-FHC position.

Position Summary: The Community Health Registered Nurse will work as part of our community based interdisciplinary primary health care team to deliver high quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health Registered Nurse who is committed to client-centred care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care for the residents of Thorncliffe Park.

Responsibilities include: Direct Client Care:  Participate in providing comprehensive primary care within the scope of a Registered Nurse in an expanded role  Coordinate client care within HATP, with community health providers, specialists and other health care agencies  Provide telephone advice and triage for requests, abnormal results, screening purposes and health prevention issues  Initiate health teaching and health promotion with clients as part of a clinical assessment and groups  Participate in a home-bound program and/or do home visits as required.


Health Promotion and Education:  Collaborate with one’s service team and other teams across the organization to identify and provide health education to target groups  Participate in community projects or agency activities as a representative of the Health Centre as needed  Assist in the development of community outreach programs based on the needs of individuals and the community  Participate in the development of, and give support to policies and strategies that are in the interest of public health and recognize the determinants of health  Incorporate health care advances into practice and while addressing the changing demographics and evolving needs of the community Organizational Responsibilities:  Participate in quality improvement, audits, accreditation and other relevant organizational committees and working groups and team meetings  Participate in the planning, implementation and evaluation of goals which are developed yearly  Plan, participate and assist in the identification of priority populations, and implement effective services consistent with HATP’s vision and values  Develop and maintain knowledge of the Thorncliffe Park community and have working knowledge of community services and resources to make appropriate referrals  Teaching and supervising nursing and medical students as needed and support volunteer placement programs as required • Participate in continuing education activities and assist in the in-service training of other employees • Other activities as assigned by the Manager, Primary Health Care

Skills and Qualifications:  Registered Nurse in good standing with the College of Nurses of Ontario  Bachelor’s Degree in Nursing  Minimum of three years’ experience as a registered nurse within a community health centre setting in the following areas preferred:  Assessing (and triaging) patients to determine the appropriate nursing and/or medical care required.  Providing chronic disease management, with an emphasis on providing care to patients with diabetes, hypertension and asthma  Administering immunizations, medications and treatments as directed by supervising physicians  Monitoring and assessing changes in patients’ symptoms and conditions and providing updates to attending physicians  Delivering and advising on women’s health screening tests, including cervical cytology tests, breast examinations and contraception  Providing travel health advice and immunizations  Skilled in assessment, health promotion & education, counseling and understanding of community development  Able to work as a member of an interdisciplinary health care team  Knowledge of and commitment to anti‐racist and anti-oppressive practice  Strong computer skills, including Microsoft Office Suite and Electronic Medical Records, preferably Nightingale On Demand(NOD)

Note: This is a bargaining unit position with UFCW. Terms may be altered as a result of collective bargaining.
