Vancouver Island Community Connections Inc is looking for a new support team member; this is a dynamic position that provides the opportunity to support two young women to live a full life in their own home. You will have the opportunity to be part of a dynamic care-giver team responsible for assisting our persons served (PS) to live successfully in both residential and community settings in an accredited agency. As part of the team you will ensure emotional, social, physical, educational and medical needs of the persons served are met. As well, you will assist PS to enhance their quality of life with activities of daily living and the development of life skills. As well you will provide positive behavioural supports and supervision in the home and community, implement and monitor individual goals through observation and accurate reporting, and participate in operating and maintaining the program of care.

Responsibilities of the Job:

With the team, participate in assessing and planning with the individuals in program; documents, implements and contributes to program evaluation. Provide life skills training such as housekeeping, personal care skills and personal finance; implements personal service plans. Assist PS with activities of daily living such as hygiene, grooming and toileting. Knowledge of g-tube, medication administration and augmentative communication. Support the two individuals in their various PS activities in accordance with care plans.

Job Qualifications:

The following education/experience are preferred and/or an equivalent combination of training and experience may be accepted.

Continuing Care Certification (LTCA, or RCA) or LPN.; Human Service Worker Certificate or Diploma, or equivalent.

Specialized training in augmentative communication and 6 months prior related experience working with people who require assistance with all activities of daily living.

A certificate, diploma or degree in related human/social service field; Or a combination of education and experience may be considered;

Preference given to those with one plus years of experience,

Current WCB Occupational First Aid, Level 1 (or ability to obtain within the probation period)

Ability to use common computer applications such as word, excel and email.

Valid class 4 BC Driver’s License, clean driver’s abstract.
