Job Description: The Continuing Care (CC) Specialist works with all programs and service areas including Long Term Care, Home Care, Transitional Care, Rehabilitation Services, Geriatric Services and Program Development. The CC Specialist is responsible for performing various key functions to support achievement of the strategic and operational direction of Continuing Care services to optimize the quality and standards of patient/resident care. The Specialist requires the skill and ability to think critically, act at a systems level, and be a change agent within the organization regarding quality initiatives and program direction. The position will be responsible for recommending direction and is integral in the planning, formulating and implementing of standards and processes with all quality and safety work in Continuing Care. This includes supporting teams of in/out-of-scope staff in project, policy and standards work. This may include quality improvement, accreditation, program guidelines and development, clinical procedures/policies, and service changes. They will make recommendations based on objectives that will directly impact bargaining unit members.
The Specialist will collaborate and partner with affiliated and contracted providers and SHA care teams, including all Continuing Care services and cross-functionally with other portfolios locally and provincially such as Clinical Standards, Quality and Safety, Digital Health, and all Integrated Health Services.
The Specialist demonstrates strong leadership to enable an innovative and positive working climate and a client-centric culture. The Specialist must work cross functionally within the portfolio and across the organization to support operations. The Specialist will develop partnerships and work closely with system and community partners.
Accountabilities
- Oversees and supports operationalization of improvement plans, service changes as identified by the Ministry of Health, SHA, Continuing Care and departmental quality improvement or strategic requirements, including impacting culture and creating environments to support change.
- Supports the development, implementation, communication and ongoing evaluation of policies, procedures and standards of practice and training for interdisciplinary teams and environments in Continuing Care.
- Leads improvement initiatives and strategies in areas such as: quality indicator corrective action planning and improvement processes, surveys, audits, accreditation required organizational practices; rehabilitation provincial strategy, system flow, program and service evaluation and recommendations, strategic planning.
- Provides coaching and mentoring to guide multidisciplinary teams and staff to promote skill development and the application and evaluations of evidence based best practice in the planning and delivery of care.
- Ensures inter-professional collaboration with health care teams/practitioners, patients, residents, families and stakeholder groups to achieve optimal health outcomes and quality of life for individuals in Continuing Care. Incorporates evidenced based practice into service delivery models.
- Engages with the Director, Provincial Continuing Care on the status of unanticipated events, needs, developments and initiatives related to quality and safety issues of patient/resident and family centered care.
- Promotes and participates in research-orientated activities for the purpose of ensuring program and service development will meet the needs of the population that we serve.

Experience
- 3-5 years experience in Long Term Care, Rehabilitation or Geriatric Services
Education
- Undergraduate degree or diploma in a Health Care related discipline or equivalent combination of education and experience.
- Licensed and in good standing with professional association and/or regulatory body, if applicable.
Licenses
- Valid Class 5 driver's license
