Job Description: The Assessor Coordinator - Degree - Senior is a supervisory, team leader position in Primary Health Care, reporting directly to the Manager. The position will provide clinical supervision for Assessor Coordinators and Social Workers on the team in the functions of case management practice. The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to coach, mentor, orientate and provide education to the team members. Drawing on standards of practice for case management; professional codes of ethics and standards of practice in health care; program policy and procedures; and, relevant legislation, the incumbent will ensure that the team provides high quality client service and professional standards of practice are maintained.

Experience

  • Two (2) years related experience
  • Experience in case management
  • Experience in leadership

Education

  • Recognized degree in nursing , or a social work degree.

Knowledge and Abilities

  • Ability to show professional maturity and leadership in clinical practice.  
  • Demonstrated knowledge and ability for competent case management practice
  • strong commitment to professional dev. & ongoing learning for self and others.  
  • Ability to work with minimal supervision
  • Knowledge of how to coach for performance
  • Makes judgements on the basis of critical thinking and problem solving criteria
  • Ability to be self-directed 
  • Highly developed written and oral communication skills
  • Knowledge in conflict resolution techniques
  • Decision making through the application of clinical, legal & ethical practice
