The Pharmacy Department is committed to patient focused care and a safe and efficient medication system as it transitions into a comprehensive automated medication management system hospital wide. Currently, the St. Peter’s Pharmacy has a computerized pharmacy system for patient medication profiles with a traditional drug distribution system that includes the preparation of sterile and non-sterile products. The Pharmacy Department is integrated across 5 sites and is supported by a regional drug information centre, trains undergraduate pharmacy technicians, undergraduate/postgraduate pharmacist students and provides a fully accredited Pharmacist Residency Program.

The Clinical Pharmacist is a member of the multidisciplinary healthcare team providing pharmaceutical care to patients in St. Peter’s Hospital, a complex care facility. The Pharmacist reviews and assesses medication orders and works with the patient and healthcare team directly to optimize medication use, avoid medication-related problems and achieve desired patient outcomes. The Pharmacist will provide drug information, promote evidence-based/cost effective drug therapy, promote clinically relevant research and be involved in educational activities for students, families and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential.


Qualifications

1. Licensed or eligible for license by the Ontario College of Pharmacists
2. Baccalaureate of Science Degree in Pharmacy
3. Demonstrated computer skills including pharmacy systems, word processing, spreadsheet and e-mail systems
4. Demonstrated ability to function as an independent practitioner
5. Demonstrated innovation and project leadership skills
6. Demonstrated commitment to continuous learning
7. Demonstrated excellence in independent practice, teamwork, change management, teaching, problem-solving and communication skills
8. Experience working within a complex continuing care facility preferred
9. Hospital Pharmacy Residency preferred
