The Clinical Care Program, Outpatient Services is seeking a full-time, permanent, Behaviour Analyst to work on the Alternate Level of Care (ALC) Transition Team. The Behaviour Analyst will report to the Manager of the Downtown West Clinic and collaborate with inpatient units and service partners in the development and implementation of discharge plans for clients who have been designated as “Alternate Level of Care” (ALC). This resource will support innovative clinical services for individuals transitioning to high support housing. The Behaviour Analyst will also provide consultations for the three downtown outpatient clinics (Downtown East, Downtown Central and Downtown West teams).

Working within a biopsychosocial approach and as a member of an inter-professional team, the ALC Transition Team Behaviour Analyst will work with clients who are currently admitted to hospital and who have been identified as discharge-ready but with significant support needs and who have been designated ALC. You will support these clients as they transition to the community and liaise with service partners to coordinate care, refer and connect clients to appropriate services and resources. The successful candidate will undertake collaborative education and capacity building with housing and community service providers. You will provide clients with an individual functional behavioural assessment and treatment strategies suited to the context of the receiving environment, or in their current living environments for the downtown outpatient clinics. The development, assessment, and evaluation of the client’s transition plan will be supported by the Behaviour Analyst. You will provide hands on support to the receiving service to implement the elements of the transition plan. You will provide team level consultation and training to facilitate successful reintegration within the community.
