Description
The University of Alberta Hospital offers a wide range of services, including cardiac sciences, neurosciences, surgery, medicine, critical care, burns, transplant, renal, and a vast number of ambulatory clinics. This position is part of the centralized Rehabilitation and Allied Health Program and will be responsible for providing social work services to the adult populations across acute care, with a focus on Neurosciences units. The candidate will be a highly motivated, confident, and dependable individual who collaborates with interdisciplinary teams, patients and their families in a busy environment ensuring trauma-informed care. The SW will hold a working knowledge of the health care system, SDoH, and community resources to support psychosocial interventions such as complex discharge planning, financial assessments, crisis intervention, decision making, advocacy, consultation with care providers, and liaising with internal/external agencies. As a Social Worker II you are part of a team dedicated to providing quality patient centered care through best practices and evidence informed approaches in alignment with AHS Values.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Minimum 2 years’ experience working as an RSW in acute healthcare settings. Current Basic Life Support (BLS-HCP) certification is required. Able to respond promptly and decisively using critical thinking and ethical problem-solving skills to analyze situations and determine appropriate outcomes. Strong professional judgment, initiative, and effective time management skills are necessary to prioritize and complete assigned tasks, from a trauma-informed lens. Maintain concise, timely and accurate communication with team, patients and families. Understanding of SDoH and health care processes to ensure advocacy and equity for patients. Experience with self-direction for independently working, and with actively and positively contributing to an interdisciplinary team are essential.
A Master of Social Work degree (MSW) is an asset. Knowledge and experience with Netcare, Connect Care and Microsoft Office is preferred. Experience with relevant legislation (including Personal Directives Act, Adult Guardianship and Trusteeship Act), decision making processes, and complex placements is an asset. Knowledge and experience working with grief and loss, and conflict resolution are assets.
