Description
Based on location availability, the successful applicant may have the option to work from either our Ponoka or Red Deer office. This position is eligible for a remote hybrid agreement after a period of orientation. Working as part of an inter-disciplinary team, you will practice from a harm reduction, recovery-based philosophy. You will act as an extension of the physician, provide direct patient care through virtual technology platforms. You will support clients with the initiation and ongoing treatment of Suboxone or Methadone and have a good understanding of same. As a SW II you will provide culturally appropriate assessment, treatment, case management, crisis intervention, education and referral services for opioid dependent clients and their families. Tasks include: monitoring prescription changes, facilitating changes with doses, monitoring urine drug screen results, ensuring the VODP physician is apprised of patient status and supporting the client’s general physical and emotional health related to addiction. Collaborating with Pharmacies, Labs, EDs, Physicians, existing AMH Services. You will take a participatory decision-making approach within a team and external stakeholders to find solutions and resources to support individuals with an opioid dependency. The successful applicant is an assertive, proactive individual with a strong foundation in mental health and addictions and excels at working professionally and independently in a collaborative, fast-paced, positive team environment that requires excellent assessment, prioritization, conflict resolution, critical thinking and decision-making skills.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Authorized by ACSW to provide psychosocial interventions. Excellent knowledge of addictions and mental illness. 1 - 2 years' experience working in an Opioid Agonist Treatment Program with knowledge of suboxone/buprenorphine and methadone. Advanced computer literacy. Above average keyboarding skills (40-50 wpm) and proficient with computer programs - Outlook, MS Office, Zoom, Connect Care, etc.
Working knowledge of Trauma Informed Care. Working knowledge of Motivational Interviewing. 1 - 2 years' experience with individuals with Mental Health and Addictions issues. 1 - 2 years' experience in counselling. Knowledge of suboxone.
