Job Description: This position works as part of an interdisciplinary primary health care team. Under the direction of the Urban or Rural Primary Health Care Manager, the Nurse Practitioner will provide primary health care services to the community. The Nurse Practitioner will be responsible for assessment, planning, implementation and evaluation of care for clients in the community both independently and collaboratively with a physician, depending upon the complexity of client needs. This includes: health promotion and protection; first line treatment and emergency services; provision of supportive counselling and instruction in management of medical conditions; disease and injury prevention; initiating and conducting community needs assessments and participating in evaluative projects; and collaboration with and referral to other health care professionals.

Experience
- Experience as RN (NP) in a Primary Health Care setting preferred.
- One year experience in community needs assessment/development/health promotion
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Registered Nurse Practitioner (RN(NP))
Other Education and Training
- Registered/eligible for registration with the CRNS as an RN (NP) or RN (GNP)
- Certification in recognized Primary Care NP equivalent program is acceptable
- Basic Cardiac Life Support Certificate (BCLS)
Knowledge and Abilities
- Demonstrates knowledge in community development/assessment.
- Knowledge of population health approach to health promotion/prevention
