Job Description: The Senior Health Educator integrates knowledge and skill reflective of Assessor Coordinator and/or Social Worker care practices in an integrated clinical services model of care. This position will apply principles of adult learning in the provision of education, research and quality client centered care. The incumbent will provide visible, proactive leadership and promote the delivery of client centered care through the application of professional standards and lean principles. In consultation with the director and managers, the individual is responsible for assessing learning needs, planning, implementing and evaluating education activities/programs, either unit based or region wide. The Senior Health Educator will use a consultative approach and research findings to provide support to individuals or groups as appropriate. The position will have specific responsibilities related to clinical knowledge, professional development and electronic systems (such as Procura, RAI-HC, SCM, EMR, etc).

Education:
- Requires a recognized Health Degree acceptable to the Employer. Must have current practicing registration with appropriate provincial licensing body.
Experience:
- Requires a minimum of five (5) years related clinical experience in the area of client needs assessment and service care coordination and/or medical social work, including case management and discharge planning. Experience in a community based setting is required.

- Demonstrates ability in teaching, facilitating and leading group activities.
- Demonstrates interpersonal and leadership skills.
- Demonstrates ability to coach and mentor personnel in a positive manner.
- Possesses effective written communication skills; strong interpersonal and verbal communication skills with patients, families and other healthcare professionals.
- Demonstrates critical thinking and problem solving abilities.
- Demonstrates ability to integrate clinical knowledge and experience into activities to enhance patient care as it relates to case management and supportive care.
- Demonstrates working knowledge of electronic systems and assessment tools.
