DESCRIPTION
Reporting to the Assistant Executive Director (AED) Miyupimaatisiiun, the incumbent oversees the organization’s primary and extended care programs and services by implementing and evaluating the activities in the following pole: Chisasibi - Wemindji - Whapmagoostui.

The incumbent is responsible for ensuring the effective organization and integration of primary and extended care programs and services. Through strategic planning, implementation, and evaluation of frontline services, they work to ensure the delivery of a high-quality client experience relationship-based service delivery framework. She/he will ensure proper planning of the human, material, financial and information resources to optimize resources for operating efficient services structured by in an integrated service framework.

The incumbent plays a key role in interdepartmental collaboration with Pimuhteheu and Nishiiyuu to ensure alignment with the CBHSSJB Strategic Regional Plan (SRP). This position is responsible for strategic leadership and operational oversight of Integrated Primary and Extended Care services delivery, ensuring seamless, multi-disciplinary care that enhances the health and well-being of individuals, families, and diverse community populations. The incumbent fosters high-quality, patient-centered care, promotes collaboration, and supports continuous improvement across all service lines. Additionally, they work closely with the AED-Miyupimaatisiiun to align clinical and operational goals with the organization’s vision, values, and healing informed practices standards. The incumbent provides a liaison function between Miyupimaatisiiun teams and the Administrative Services Group to address operational issues and to resolve problems.

The incumbent provides liaison and communications with all concerned partners of the communities, in and outside of Eeyou Istchee, as directed by the AED - Miyupimaatisiiun, and represents the AED - Miyupimaatisiiun in various circumstances.

SPECIFIC FUNCTIONS
Strategic Responsibilities
1. Acts as a strategic partner in shaping the organization's plans and programs, with a particular focus on their impact on Integrated Primary and Extended Care Services, which includes Multi-Services Day Center, Cree Home and Community Care Program, Pharmaceutical Dispensaries, Allied Health, Psychosocial and Current Emergency Acute services and where applicable Robin’s Nest Shelters.
2. Translates strategic organizational plans into Miyupimaatisiiun tactical and operational action plans within their area of responsibility.
3. Contributes to specific regional responsibilities, and the leadership, accountability, planning, coordination, organization, development, evaluation, and the identification of resource development needs for the Integrated Primary and Extended Care Services and Programs.
4. Ensures the planning, implementation, supervision, control, performance, quality assurance and reporting, of the primary and extended care services and programs of the organization.
5. Collaborates in the preparation of policies and procedures pertaining to primary care resources management within their area of responsibility.
6. Contributes to the planning and development and evaluation, and ensures implementation includes measures to support healing-informed practices within their jurisdiction according to the CBHSSJB’s clientele approach.

Delivery of Primary and Extended Clinical Care Services and Program Responsibilities
7. Ensures the delivery of clinical programs and services are consistent with the CBHSSJB Vision, SRP strategic orientations, Codes of Ethics, applicable legislation and regulations, best practices, and measures for quality assurance and cultural safety.
8. Collaborates with the implementation of Telehealth Services and associated training within their jurisdiction.
9. Oversees the development and implementation of initiatives aligned to the Miyupimaatisiiun Direction’s mandate, such as a new MSSS programs and special project, to improve the health and wellbeing of the Cree population.
10. Leads the adaptation of programs and services according to the human resource capacity and the needs of the Cree population accessing services.
11. Ensures that the policies, procedures, codes, quality and practice standards, legislation and regulations, individual and collective contracts, and directives within their area of responsibility are respected.
12. Ensures the quality of clinical programs and services offered under their leadership with the support of the Miyupimaatisiiun Direction management team, Directors of Professional Services (Health, Allied Health and Psychosocial) and Council of Physicians, Dentists and Pharmacists.
13. Supports the Directors of the CMCs in the implementation of mechanisms to supervise and evaluate the activities performed by professionals and associated paraprofessional staff members.
14. Leads the implementation of key performance indicators aligned to healing informed practices, norms and standards of quality in conjunction to client experience and satisfaction.
15. Ensures the organizational process of reporting incidents, accidents, sentinel events and near misses related to clinical programs, services and activities of professionals is respected to support the quality and risk management system for client safety.
16. Collaborates in the development of clinical professionals and teams to create a collaborative and integrated model of care that is reflective of Cree culture.
17. Ensures the circulation, application and compliance with the CMC clientele and their families’ individuality, security, rights, code of ethics and privacy and confidentiality.
18. Oversees the establishment and execution of effective mechanisms to supervise and assess clinical activities carried out by professionals and paraprofessional staff within her community. This includes implementing organizational systems for preparing intervention plans, maintaining client records, and safeguarding confidential data through meticulous record-keeping practices.
19. Leads or supports various Miyupimaatisiiun ad hoc working groups, internal and external committees, as assigned by the AED - Miyupimaatisiiun where necessary to ensure proper representation.
20. Ensures direct support to the CMCs from the Regional Services by providing the following:
a) Information and training for new programs, expertise for navigating complexities of healthcare system, and collaboration in program coordination committees;
b) Advice, coaching, and clinical and programming support to the line managers for the implementation of the integrated model of care service delivery;
c) Support in operational delivery planning, organization and coordination for program implementation; and
d) Facilitate and coach to support the implementation of integrated services, interdisciplinary teams, and multidisciplinary teams.
21. Carries out strategic files and special projects according to the direction’s mandate and as assigned by the AED - Miyupimaatisiiun.
22. Plays a key role within the Miyupimaatisiiun Direction management team, actively contributing to strategic planning and leading change management efforts. Drives the implementation of improvement initiatives to enhance service quality, optimize operational efficiency, and ensure a high level of client experience and satisfaction.

Proximity Services and Key Partnerships
23. Collaborates with the Pimuhteheu Group to incorporate the methods and tools of Nishiiyuu Miyupimaatisiiun and Traditional Knowledge in parallel to clinical frontline service delivery, and for the development of program models that support a seamless integrated care within a multidisciplinary approach.
24. Collaborates in the development of local and regional area networks of integrated medical services in the region and in the creation of linkages and corridors of services for secondary, tertiary, specialized and super-specialized services outside of the region including midwifery services and other initiatives.
25. Ensures the collaboration protocols with local and regional community service providers are followed to enhance service delivery and minimize risks.
26. Ensures human, financial, material, and information resources management and the general administration of the CMC.

REQUIREMENTS
Education and experience
• Bachelor’s degree in administration or science or with a health or social related specialization.
• Ten (10) years’ experience in management in a health or social services setting or public organization.
• Master’s degree in health or administration or public affairs is an asset.

Knowledge and abilities
• Strong knowledge of the MSSS and Quebec Healthcare programs, legislation and regulations, the RUIS Network, orientations and trends especially for integration of traditional methods of healing into Healthcare surveillance, protection, promotion and prevention.
• Strong knowledge of public health current issues and trends, and program planning and supervision, including the development of policies and programs, and quality assurance systems.
• Good knowledge of public health surveillance methods, techniques and statistical analysis.
• Good knowledge of public health communication needs and approaches.
• Knowledge of Cree culture, communities and language is an asset.
• Strong knowledge of current issues and trends in human resources management, operational delivery planning, organization, coordination, supervision and quality assurance systems.
• Strong leadership and line management skills, including finance, material and information resources management.
• Ability to effectively collaborate with departments staff/functional in a line-staff organization.
• Ability to both manage the planning and development of new programs and services and maintain quality assurance of existing programs and services.
• Ability to lead and manage the planning and development of new clinical programs, services, supporting protocols and to maintain consistent application of decision principles.
• Excellent record in health and social services management.
• Knowledge of, or ability to grasp the issues and context that relate to First Nations health and social services programming.
• Knowledge of accreditation standards and processes.
• Ability to apply Eeyou (Cree) culture and values in the workplace.
• Experience and knowledge in developing holistic health and wellbeing programs and services.
• Experience and/or ability in establishing and maintaining good relationships with the Council of Chishaayiyuu, elders and traditional healers, as well as community residents and leadership.
• Excellent critical thinking, synthesis, decision-making, planning and organizational skills.
