Reporting to the Area Manager, Red Deer Home Care, you will interact with the team and clients/families to provide quality care to clients in the community. As part of the Intensive Home Care and Community Response team you will work closely with clients/families and members of the health care team to facilitate early hospital discharge and to prevent hospital admissions when appropriate. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates. Working in collaboration with patients, families and health professionals, this position helps support transitions throughout the health care system. The successful candidate will provide assessments and education for clients and caregivers, provide direct care as required, liaise with Case Managers, and play a key role in timely and appropriate referrals to other resources as required.Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Minimum 3 years community (Home Care) experience required. Strong knowledge of Home care program, the acute care environment, community resources, and stakeholders is required.
Ability to provide case management support is an asset. Experience with HPT, wound care, client teaching are asset. Demonstrated excellent communication skills in complex and dynamic environments. Ability to work independently as well as being a contributing member of the team.
